GENERAL SUMMARY:
The Patient Care Assistant is a temporary position for employees hired into the Patient Care Technician Training Program without experience or training as a Certified Nursing Assistant. The PCT training program is 120 hours in length, inclusive of classroom, skills lab and clinical hours. During PCT training, the Patient Care Assistant may assist the clinical staff in the provision of patient care with continual supervision. These duties may vary by shift or may be unit-specific. The Patient Care Assistant is trained in the provision of care for patients of diverse ages. Works rotating shifts, weekends, and holidays as scheduled.
ESSENTIAL FUNCTIONS:
- Attends didactic and clinical training sessions as outlined in the course curriculum. Adheres to attendance requirements.
- Receives training to assist patients with activities including personal hygiene, bathing, ambulation, transporting, dressing/undressing, nutrition/feeding, and elimination needs.
- Assists in the delivery of patient care with supervision by an RN, LPN, or Patient Care Technician.
- Responds to patient calls and performs patient rounds to ensure the environment is safe/tidy.
- Trained in the use of safe patient handling techniques when repositioning, transferring, and ambulating patients.
- Performs safety patient observation for patients who are impulsive and/or at risk of injury to self or others.
- Assists with post-mortem care.
- Observes and communicates patient needs to the nurse/PCT.
- Collects and labels specimens.

Job Summary
Coordinates the care for Hospice patients who reside in the Hospice House facility. Coordination of care includes: coordination and direction of the interdisciplinary team, patient, family and facility staff; patient symptom management; and assessment of patient and family physical, psychosocial, cultural and spiritual needs. The incumbent provides end-of-life education to patient, family and facility staff and advocates for patients and families to ensure patient-focused care.
An incumbent exercises independent judgment consistent with professional licensure and applies skills obtained through academic and clinical preparation. Unusual problems or proposed deviations from guidelines, practices or precedents are discussed with a manager or physician before being initiated.
We are looking for a responsible and compassionate RN with top-notch clinical skills, a caring attitude, and the ability to work independently and act as a Patient Care Coordinator (PCC) to suport the provision of effective, high quality patient care. It is also important to display excellent verbal and written communication and interpersonal skills as well as the ability to effectively educate patients and their families on the in-home healthcare process.
As a Patient Care Coordinator RN , you will be responsible for managing the highest quality patient care according to the specific orders of each patient’s individual physician. This will involve, utilizing your knowledge and skills to educate patients and their families on prevention and healthy habits; help to keep patient treatment plans organized, educate patients about their medical conditions, help to evaluate their progress to recovery and assist patients to connect with other health care providers. The Patient Care Tech is a multi-skilled, non-licensed health care provider who provides direct patient care under the direction of a registered nurse. Applies knowledge of growth and development to perform detailed assignments/interventions. Provides consistent care based on the changing physical and psychosocial needs, unique to the patient.
Responsibilities of the Patient Care Tech include but are not limited to the following:
- Operate the following equipment: Computer, automated vital sign machines with recognitions of abnormal vital signs and timely reporting, O2 saturation, glucose meter, intercom, telephone, patient care equipment required for daily care, scales and lifts
- Basic EKG form monitoring and operation of machine and the recognition of life-threatening dysrhythmias/subsequent care, if required by area/ location
- Perform nursing care and duties delegated by Registered nurses
- Venipuncture, discontinuing INT’s
- Perform 12 lead EKG, based on area/ location
- Good telephone and communication skills
- Excellent customer service skills
- Ability to adapt to changes in health care technology.
