Amazing Job Opportunity:  Franciscan Hospice and Palliative Care is  looking to grow our outreach in the Hospitals within our service delivery areas (King, Pierce, and Kitsap counties) by having clinical liaisons back in the field.  There are currently 3 positions open.  While each position will be assigned a primary hospital to base out of, there is a high likelihood that the RN  will have the joy of going to other hospitals and/ or offering support remotely to assure transitions of the patients leaving to go home from the hospital.  Our goal is to assure that every patient has a soft landing to our services,  that the family agrees with the hospice philosophy, understands the benefit of their loved one coming onto our services, and that the hospital teams  feel we offer the best in customer service when it comes to getting the patient out of the hospital in a safe and timely manner.    In addition, we want to continue to articulate the benefits of FRANCISCAN FIRST .    If you are interested in using your education and advocacy skills to promote Franciscan Hospice in CHI and non CHI hospitals and support those patients at this critical juncture of their lives, please reach out to Jennifer Sutton at jennifersutton@chifranciscan.org.    The successful candidate will be someone who loves meeting new people, able to slide quickly between talking with a MD/discharge planner about a potential referral into having one of those tender conversations with a family who is newly exploring their next care options.   Enjoying being able to jump in your car is a plus, and days could easily be a mix of movement between floors and locations.  You are part Cicely Saunders -  wanting people to know they matter to the end of their lives,  part Sherlock Holmes – looking for the eligibility criteria,  and  part  educator  as you continue to teach hospital staff/physicians about the benefits of our programs both to the patient but also in in regards to the hospitals own goals of  preventing readmissions and reducing the mortality rates. When surveyed, over 70% of the general population doesn’t want to die in the hospital, they want to be home surrounded by their loved ones – you can help them achieve that goal at this critical time in their life.
