This would be a night position with shift differential working 3 shifts from 7p to 7am.   Work is performed under the supervision of the Sleep Lab Director and requires thorough knowledge of the function and safe operation of equipment used to monitor and record breathing (airflow/effort), electroencephalograms (brain waves), electrocardiograms (heart rate and rhythm), electromyograms (muscle activity), and pulse oximetry (blood oxygenation / saturation).     ESSENTIAL FUNCTIONS: -  Laboratory set-up -  Patient preparation -  Performing, observing and evaluating daytime and overnight polysomnograms, -  Staging / scoring sleep study -  Assisting in the tabulation of data from scored records, and -  Monitoring and checking equipment for possible changes/adjustments as necessary during the study - Gather and analyze patient information. Job Summary: The Population Health Outreach Specialist works as an integral part of an interdisciplinary team to proactively identify patients due or over-due for specific health services. The Outreach Specialist contacts patients, motivates patients to complete recommended health care services and assists in scheduling appointments. The Outreach Specialist supports Population Health patients by closing gaps in recommended clinical care, performing chart reviews, and managing the disease registry.   Essential Position Expectations: - Utilizes standardized tools and scripting to conduct patient outreach to patients to schedule and coordinate preventative health care services such as Annual Wellness Visits, cancer screenings, immunizations, and chronic disease interventions. - Conducts pre-visit chart review focusing on closing gaps in care and complete, accurate risk coding. - Manages CMS web interface quality reporting annually. Supports additional quality reporting requirements of PHSO contracts as needed. - Conducts outreach to patients with medication adherence deficits utilizing the PHSO’s standard medication adherence patient assessment. - Identifies and refers high risk patients to the PHSO ambulatory care management team for care coordination services. - Practices motivational interviewing to support health behavior change. - Requests documents from outside facilities needed to support care gap closures. - Oversees the PHSO’s disease registry assuring the database is up to date, accurate and conducting manual data entry when required. - Accepts referrals from care management team members to assist with the above position expectations. - Answers patient questions regarding appointments and/or testing. Addresses potential barriers to care and collaborates with Provider to answer patient questions and concerns that fall outside of scope. - Documents all patient interactions in the patient’s health record or care management documentation system immediately after the call or meeting is completed. This position promotes and monitors utilization of MercyOne wrap around services; and works to prevent avoidable ED visits and hospitalizations.   Essential Position Expectations: - Provides care management and care coordination services for Population Health patients receiving services in a swing bed, Skilled Nursing Facility, Inpatient Rehab Facility, or Long-Term Acute Care Hospital. - Determines the goal Length of Stay based on primary diagnosis for skilled need and communicates the goal to the post-acute care facility contact. - Communicates with care team and/or patient twice per week. - Attends facility’s weekly multi-disciplinary meeting in person/via phone/virtually or request an update if unable to due to conflicts. - Attends or initiates Patient Care Conferences as needed. - Routinely conducts PHSO’s standard Functional Assessment Survey to document functional status and progress towards goals. - Actively coordinates discharge planning with care team, providers and patients encouraging utilization of MercyOne wrap-around services. Monitors progress towards discharge and works proactively to overcome identified. barriers/concerns for discharge. - Applies motivational interviewing to strengthen the patient’s own motivation for change and movement towards health behavior goals. - Round on patients and provide coaching and education to support self-management. - Facilitates warm hand-off transition to Primary Care Provider and PHSO RN Transition Health Coach by providing medication list, discharge summary, and any appropriate additional items. - Participates in quality improvement, measurement and data collection to include populations with clinical and financial risk. - Delegates to support team such as community health workers, as appropriate. - Informs primary care provider of ongoing care management activities, patient progress and changes in the patient’s health status. - Documents all patient interactions in the patient’s health record or care management documentation system immediately after the call or meeting is completed.
