JOB SUMMARY: In accordance with our mission and core values and under the supervision of Manager-Behavioral Health, the Behavioral Health Therapist Coordinator will review the workload assignments of the Behavioral Therapists and Discharge Specialists. The coordinator will utilize advanced practitioner skills to intervene in complex patient care situations. The coordinator will assess, evaluate and train therapists and discharge specialists to achieve a consistently high quality of excellence in Behavioral Health. The coordinator will act as a resource for physicians, nurses and other departments in the hospital when intervention is required. The coordinator will have input into program planning program evaluation. The coordinator will also participate in the interviewing process for therapists and provide feedback for the evaluation process.
ESSENTIAL FUNCTIONS:
- HUMAN RESOURCES MANAGEMENT
- Works with Behavioral Therapists on a daily basis to insure optimum productivity and quality of care.
- Facilitates staff completing assignments within the designated time limits.
- Utilizes staffing in the responsible manner both fiscally and clinically.
- Participates with the manager in the interviewing, hiring, and orientating of new therapists, as well as probationary and annual performance evaluation.
- FISCAL MANAGEMENT
- Assists in the development of policies and procedures, and training in any new local, state or national standards, policies or laws.
- Serves as a resource for all customers in regard to standards and best practices within own area of expertise.
- Helps project capital needs for operational and capital budgets.
- Works with behavioral health manager marketing program and services through community outreach.
- Serves as the indigent liaison to all community mental health centers throughout the state, ensuring that appropriate information is disseminated to secure authorization for payment. Troubleshoots potential problems with community mental health centers concerning these clients during admission and prior to discharge.
- MANAGEMENT OF INFORMATION AND SUPPORT
- Maintains an understanding of the department's processes and effectively communicates information to the staff.
- Serves as mental health court liaison. Attends court as needed for our patients. Serves as an expert resource for facilities throughout state when filing proper documentation for various involuntary admission procedures.
- Maintain the indigent day spread sheet.
- Update the Behavioral Health Census.
- Effectively works with staff to enhance professional development and job satisfaction.
- SELF-DEVELOPMENT AND LEADERSHIP
- Maintains current professional requirements according to department and Health System standards, by completing hospital mandatories and department competencies in timely manner.
- Actively participates in individual and group educational training activities.
- Assists in writing or revising procedures to keep manuals current and meaningful for staff use.
- Assists in setting standards for performance in Behavioral Health Adult Care.
- Assumes preceptor role in orientation of new employees and students
- Pilots and implements new programs to improve patient care as approved
- Serves as a resource to Emergency Department when behavioral health patients present for psychiatric care.
- Coordinates the intern program. Assuring that they are properly assigned and supervised after approved by nurse manager.
- Attends career and education fairs at various schools and facilities representing St. Vincent Behavioral Health.
- Takes responsibility for helping to staff department during busy times or absence of therapists by covering caseloads and doing extra therapy groups as needed.
- Adapts to change in a positive, professional manner to meet the demands of the changing Health System environment.
- Manages a maximum caseload of 2-3 patients.
- Rotates Behavioral Health call with other therapists
- Helps assure staff competency by providing classes, in-services, and one-on-one instruction as needed.
- Able to communicate the goals and values of the hospital to the staff in an effective manner.
- Assists in staff meetings to discuss new processes and the barriers encountered in implementing them.
- Promotes professionalism of Behavioral Health Department and Behavioral Therapists.
JOB SUMMARY: The Intake Coordinator is responsible for intake processing by receiving written and telephone requests for service; communication and documentation of patient information; processing referrals; coordination of admissions, does this from the hospital referrals; managing patient information systems; verify insurance and initiate authorizations; teamwork; interprets Medicare and Medicaid requirements and regulation. Must adhere to all rules, regulations, and standards of accrediting and regulatory bodies.
ESSENTIAL FUNCTIONS:
1. COMMUNICATION AND DOCUMENTATION
Receives and documents information on new and current patients via telephone and
fax machine from physicians and hospital staff.
Conducts telephone interviews to obtain patient information from referral source.
Establishes rapport with new physicians and obtains physician’s UPIN number.
Coordinates activities with RN, obtains guidance when indicated.
When necessary, coordinate patient placement at other Behavioral Health providers.
Communicates patient information and bed number to the PIC.
Maintains confidentiality per HIPAA standards.
2. PROCESSES REFERRALS
Effectively processes referrals by performing clerical functions to assure patient
Admission is made in timely manner.
Determines funding source, verify insurance and initiate authorizations.
Makes referrals to other community resources if service is not provided by Behavioral Health.
Checks patient data to see if the patient has received previous service, and records pertinent data.
Records referral data on Intake Log.
Process reports and statistics as requested.
3. COORDINATION OF ADMISSIONS FROM HOSPITAL REFERRALS.
Effectively coordinates admissions and referrals of new patients from hospital patients with appropriate nursing team.
Completes appropriate Intake paperwork.
Calls team and attaches Intake paperwork to Admissions packet, for pickup.
Assists case management as needed with referrals.
4. INFORMATION SYSTEMS
Enters patient demographic and insurance information into computer.
Process reports as requested.
5. TEAMWORK
Participates in team meetings and inservices as required.
Follows Agency policies and procedures in all aspects of the position.
Participates in educational programs to maintain professional skill level.
Maintains confidentiality of client and employee information per agency policies.
Assist with Peer Education and orientation as assigned by Clinical Director.
6. INTERPRETS MEDICARE AND MEDICAID REQUIREMENTS AND REGULATIONS TO PATIENTS AND REFERRAL SOURCES.
Takes inquiry calls for behavioral health services.
Interprets Medicare, Medicaid and private insurance requirements to patients and referral sources.
Screens inquiries/referrals for pay source and needs.
Route inquiries/referrals to appropriate teams, services, or agency.
PRINCIPLE RESPONSIBILITIES:
Assists the primary care physicians in recognizing, treating and managing mental health, substance use, and psychosocial issues.
Contribute and participate as a member of the primary care team.
Provide consultation and education/training to the primary care providers and staff to enhance their skill and effectiveness in treating mental health problems.
Provide brief, focused interventions for clients in need of mental health and substance use services within the primary care setting.
Advise and assist the primary care provider about the best place of service for patients with mental health/chemical dependency problems, either in the primary care setting or referral to specialty treatment.
Conduct brief screening or intake interviews to determine diagnoses and functional assessment to work with primary care physician to create and implement integrated care plans.
Provide crisis intervention services when required.
Assist in detection of high risk patients and develop collaborative plans to prevent worsening of conditions.
Work in conjunction with primary care provider, where indicated, to effect behavioral changes in patients with, or at risk for, physical disorders/medical conditions to help them make healthier lifestyle choices.
