CHI Health Partners (CHP) is a physician-driven, clinically integrated network initiative that builds on the strengths of participating providers to improve patient health, increase efficiency and enable physicians to succeed in today’s changing health care payment and delivery environments.   To learn more about CHI Health Partners, please visit our website at https://www.chihealthpartners.org/   This opening is for an entry level position.   With guidance and oversight from the Population Health Technology team, some of the key functions that this role will perform are:   1)  Support of the data flow, data structure and master table maintenance for the Population Health Technology application 2)  Aiding in auditing and facilitating the data interchanges between data systems and stakeholder groups in support of the Clinically Integrated Network objectives 3)  Providing end user support for users of the Population Health Technology tool 4)  Partner and collaborate with CHP's Population Health Technologies team to implement data architecture and design, in order to support analytics 5)  Under direction, assisting in producing and summarize recurring reporting of data to key stakeholders 6)  Learning the source data systems and population health technologies used by operational leaders to support the goals of the organization
