Identifies breastfeeding experiences that are not the norm (e.g., improper latch-on, inadequate intake, etc.) and makes an immediate and appropriate referral (e.g., Lactation Consultant, other healthcare provider, or outpatient follow-up).    Completes all required documentation in a timely, accurate, and thorough manner in accordance with FHS standards and procedures.   Provides education and community outreach on breastfeeding, including conducting or assisting with group classes, in-services and presentations to patients, healthcare providers and interested community organizations. - Assists in course development, revision and evaluation processes.  - Acts as a liaison/representative of the Franciscan Health System and Women’s Care Centers to care providers, community agencies, other FHS task forces and committees as requested.  - Attends staff meetings and stays up-to-date on breastfeeding best practices.  Cultural Sensitivity and Competence:  Demonstrates proper use of communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services.  Patient Population Served:  Demonstrates knowledge and proper skills associated with the department’s defined specific populations served.   Performs related duties as required.

Pay Scale: $30.97 - $41.77   Shift: Casual Part Time, Varied   Job Summary:   The Licensed Clinical Social Worker (LCSW) conducts psychosocial assessments, determines patient and family needs, and develops a plan of treatment consistent with meeting those needs. The LCSW acts as a consultant to other disciplines. He/she interprets psychosocial information to patient, families and other professionals and works to link patients and their significant other to community support systems.   Essential Duties: - Conducts psychosocial assessments on referred Home Health/Hospice patients and develops a plan of treatment consistent with standards established under Medicare or other third party payors. - Provides short term therapy and crisis/grief counseling to selected Home Health/Hospice patients and their significant other. - Maintains a current and complete knowledge of local, state, and federal agencies and resources, acts as a liaison between these agencies and - Home Health/Hospice staff and patients, and completes referrals as appropriate. - Responsible for coordinating and implementing discharge plan from Home Health Services for specific patients as appropriate. - Provides consultation services to other disciplines, evaluating and interpreting psychosocial information to other professionals. - Completes documentation as required by Medicare, other third party payors and The Joint Commission. - Provides field supervision for Clinical Social Work Associate as required by state regulations.
